Service Terms

This practice does not provide crisis mental health care services. If you require urgent assistance at any time please contact an appropriate emergency/crisis service. Contact between appointments is limited to appointment scheduling only. Whilst services provided may assist with problems you are experiencing, there are no guarantees regarding the outcome/s.

Services are not rendered in public hospitals.

Payment Terms

This practice requires that your account be paid on the day of service (unless by prior agreement invoices are issued post consultation requiring payment within 7 days). Services will be suspended indefinitely in the event of non-payment or late payment.

Types of payment accepted:  Visa, MasterCard, Direct Deposit.

Cancellation Policy

Failure to attend a scheduled appointment without adequate prior notification will result in the client being billed a cancellation fee. This practice requires 1 business day’s notice when cancelling and/or rescheduling.

Client Privacy

Your personal details and heath information is only collected directly from you or your authorised representatives; held, used or disclosed as is necessary for the provision of health services at this practice. Client notes are held on Halaxy which is a secure practice management software. For more information regarding Halaxy please see the Halaxy Privacy Policy. Information may be used or disclosed for the following reasons:

• For communicating relevant information with other treating practitioners and support workers, such as; general practitioners, private psychiatrists, case managers, support coordinators, residential support workers or other involved health professionals as deemed necessary for ongoing health service provision.
• For follow up reminders/notifications/information relating to your scheduled appointments.
• Auditing/Accounting/Medicare/Health Insurance/NDIS procedures.
• For legal related disclosure as required by law (e.g. subpoena, other court order, suspected child abuse).
• To lessen or prevent a serious and imminent risk to harm yourself and/or serious threat to another person/s safety.

You can request access to and/or seek correction of your health record verbally or in writing. All electronic and hard copy health records are stored securely to protect information from misuse and loss and from unauthorised access, modification or disclosure. Information is not likely to be disclosed to overseas recipients. If you feel there has been a breach of your privacy you can complain directly to the practitioner, whom will comply with applicable guidelines issued by the Office of the Australian Information Commissioner for handling such a complaint.
